The City of Manhattan Beach’s Older Adults Program is extending the deadline for the current Needs Assessment Survey. Surveys may be turned in to the City of Manhattan Beach until March 31, 2009.
The Older Adults Program has inserted a survey into the spring 2009 MANHAPPENINGS activity guide, requesting all persons age 60+ to fill out the survey to help improve the Older Adults Program and services. The survey asks questions about activities and community service, transportation needs and volunteering activities. If you have not received a survey or need an extra one, you may download one below. The downloadable survey below may be filled out on the computer or printed out and filled in by hand.
Older Adults Program Survey
You may drop off your survey at:
- City Hall - Parks and Recreation Department (1400 Highland Avenue)
- Joslyn Community Center - Older Adults Program Office (1601 Valley Drive)
- In one of the YELLOW CITY-PROVIDED MAIL BOXES located at:
- City Hall
- In the post office parking lot (corner of 15th and Valley Drive)
- Creative Arts Center parking lot (1560 Manhattan Beach Boulevard)
