What Services Will Keep You Independent at Home?

Independence at Home
Independence at Home (IAH) is a SCAN community service that provides no-cost programs, support, and resource referrals for older (55+) adults who do NOT have SCAN health insurance. These services are designed to help these individuals maintain their health and well-being in the community. Care management is provided by experienced social workers, nurses, and therapists who direct the provision of necessary services for qualified seniors.SCAN Independence at Home logo

Independence at Home’s Goals:
The goals of the (IAH) Program are to keep seniors and their caregivers healthy and independent by:

  • Providing direct services to help older adults live safely in the community. 
  • Building access to programs and services vital for physical, emotional and social wellbeing. 
  • Invest in the community through support of community based organizations and partnering with city, county and statewide efforts in key areas impacting older adults.

Beach Cities Health District
Beach Cities Health District (BCHD) provides services to improve the quality of life and maintain the independence of older adult residents and residents with disabilities within the Beach Cities.Beach Cities Health District logo

A Plan To Age Successfully At Home:
Care Management provides confidential in-home visits and assessment along with recommended care plans, direct services and frequent follow-up to support independent living at home while staying connected to the community from a professional Care Manager. Care Management is available at no cost for older adults (ages 60+) and adults with disabilities (ages 18-59) living in Hermosa Beach, Manhattan Beach or Redondo Beach.
